E-mail: [email protected] A 67-years-old woman with diabetes mellitus was admitted because of having difficulty in opening her left eye for two days following a left frontal pain of two months duration. Neurological examination revealed a left trochlear nerve palsy and a left oculomotor nerve palsy with preserved light reflex. Tendon reflexes in the limbs were equally depressed. All sensation was impaired in a stocking and glove fashion. Magnetic resonance images (MRI) of the brain with gadolinium showed a mass lesion in the left sphenoid sinus (Figs. 1A-B), and two brain abscesses in the left medial temporal and right temporal-occipital regions (Fig. 1C). The mass was removed surgically and pathological examination revealed clumps of aspergillus with septated hyphae and focal purulent exudate. Cerebrospinal fluid analysis was unremarkable. The patient received a 2-month course of combined intravenous amphotericin-B and oral itraconazole. Oral itraconazole alone was continued for 10 more months. The functions of left oculomotor and trochlear nerve recovered after above treatment. Two months after admission, slight right hemiparesis was observed. MRI of the brain showed thrombotic change of the siphon portion of the left internal carotid artery (ICA) (Figs. 2A-B) along with a low-flow infarcInvasive Aspergillosis and Thrombosis of Carotid Artery

Carotid artery thrombosis and cerebral infarction after multiple traumas
Carotid artery thrombosis and concomitant brain infarction after blunt trauma are rare conditions. We report a 34-year-old woman with multiple traumas due to pedestrian car accident. At the time of initial visiting, she was somnolent and her Glasgow coma scale (GCS) score was 14. Initial vital signs (V/S) and brain computed tomography (CT) scan were normal. One day after admission to the emerge...

Introduction: Penetrating trauma to anterior neck can induce cerebral ischemia due to carotid artery injury. Brain ischemia also can present after surgical carotid repairs. Early diagnosis and suitable treatment modality prevent from permanent neurologic deficit post operatively. Case Report: A 30 years old man with stab wound to zone two left side of neck underwent exploration and penrose i...
متن کاملCarotid artery thrombosis treated with catheter intervention using proximal occlusion and flow reversal.
We present a case of symptomatic carotid artery thrombosis treated with catheter intervention under proximal occlusion and flow reversal embolic protection. Although catheter intervention is contraindicated in carotid artery thrombosis due to the risk of distal embolization, the introduction of proximal occlusion embolic protection devices allow interventionalists to use catheter intervention w...
متن کاملPresentation of a Non-invasive Method of Estimating Arterial Stiffness by Modeling Blood Flow and Arterial Wall Based on the Determination of Elastic Module of Arterial Wall
Introduction: Arterial stiffness is an important predictor of cardiovascular risk. Several indices have been introduced to estimate the arterial stiffness based on the changes in the brachial blood pressure. Since the substitution of the blood pressure changes in the central arteries such as carotid with the blood pressure changes in the brachial results in error in the blood...
متن کاملTreatment of invasive aspergillosis with itraconazole.
PURPOSE Invasive aspergillosis in the immunocompromised host is one of the most difficult therapeutic problems. Itraconazole, a new oral triazole, is inhibitory as well as fungicidal against Aspergillus species in vitro. It is active against Aspergillus infections in animal models. We present our experience with itraconazole therapy of 21 patients with aspergillosis. PATIENTS AND METHODS Eigh...
